General CV prevention is of the utmost importance should be multidisciplinary. Best medical therapy (BMT) includes CV risk factor management, including best pharmacological therapy, as well as nonpharmacological measures such as smoking cessation, healthy diet, weight loss and regular physical exercise. As of today , a large number of patients are wandering in the Basilicata region unaware of their burden of risk, until a CV prevention. Often these patients are discharged without a multidisciplinary assessment of CV risks factors and a proper therapeutic strategy for secondary CV prevention. There are no available data on the strategies adopted in each hospital to achieve CV prevention and there are no data related to physician adherence to guideline recommendations or patients adherence to physician prescriptions.
The project aims to set up, as recommended by available guidelines, a multidisciplinary Cardiometabolic Team to make decisions for the management of patients with diabetes mellitus (DM), Chronic Kidney Disease (CKD) and peripheral artery disease (PAD).
This will allow to establish an institutional therapeutic pathway that with allow any patient to receive a proper prescription for CV prevention.Patient’s adherence to physician prescriptions will be assessed through an appropriate follow up.
A follow-up assessment, will schedule the dates of the visit, of the diagnostic and of the clinical test that should be performed and evaluating therapeutic adherence.
In general the Cardiometabolic team will counsel to each patient wit the recommendations in order to achieve an optimal prevention strategy in order to reach a correct guidelines recommended LDL-C therapeutic regimen.
The ultimate goal of this project will be to improvement of care of DM, CKD, PAD patients. In particular we will increase the awareness, among the treatment population, that lifestyle changes are mayor determinants to achieve optimal risk factors control and together with a properly prescribed medical therapy they are able to minimize cardiovascular events. Furthermore we will evaluate adherence to prescription in this peculiar patient setting, an information that, differently from other clinical settings (Acute coronary syndromes or cardiovascular revascularizations) is not currently available in the literature.
